Once someone goes through all the steps to set up a name change, there should be a PM being sent out to the player who requested it from whoever changes it.
If Yes, Just make sure it's something simple saying it has been accepted.
or if it's no, Add a small but smart message on why it is not.

If you request to send a name change, and it doesn't get accepted, there is no way to know unless you ask the admin/person who denied the request.
Just an easy way to know what happened pretty much.
