1. You were banned for "Hacking/Toxic[ity]" which lasted 3 months.
2. You were then VAC banned for cheating on CS:GO then you got another account and joined Jailbreak, causing an admin to ban you for 6 months for ban evading (when you were actually just getting another CS:GO account because of your previous VAC ban, saying this however; you should've known not to come on the server when you're banned).
3. You leaked personal information on the forums of one of our members, resulting in a permanent Forums ban but someone forgot to update the ban on sourcebans (allowing you to join the server after the 6 months).
Saying the above though, I believe you've waited enough time to be unbanned, with no incidents since the personal information incident.
