So as disabling PrioritySpeaker has worked (no more mutes), I've concluded this was caused by the SourceComms update:

I've reworked the Priority Speaker component of WardenTools and it now uses SourceComms to do session mutes and the unmuting logic is now (dare I say it) perfect:

Anyway, I'm pretty confident this will fix it but theres only so much you can test solo with a extra laptop and bots.
I'll leave this open for a bit for confirmations, then I'll close it as fixed. 1-2 days should be enough.
